Following improvements made to discharge planning tools earlier this spring, as of June 16, 2026, a workflow for ordering conditional discharge will be available for most* Connect Care facilities.
Once a patient is medically ready for discharge, a conditional discharge can be indicated with a prescriber order (linked to a discharge order) that specifies conditions that must be met for the discharge to proceed, such as completing physiotherapy milestones. Such conditions should be clear, simple, and narrowly defined; if such precision is not possible, then a conditional discharge order should be deferred until clarity emerges. Conditional discharge orders allow for earlier discharge without the need for the most responsible provider to be present.
Conditional discharge orders can be entered via Discharge to Home, Out-of-Province, and Inter-facility transfer to non-Connect Care facility navigators, and the order panel is also available via the Orders activity. See the QuickStart for an overview and the tip sheet for further details.
*At this time, the conditional discharge order workflow is not supported when the destination healthcare facility uses external pharmacy services supported with “ePrescribe” functions (sites listed here). Until further development makes ePrescriber conditional discharge orders possible, users are advised to prepare for such transfers, when needed, by entering a simple discharge order that is future-dated.
