- Transitions of care (from one provider, team or site to another) can occur within (intra) or between (inter) facilities. This session explores workflows when physicians hand over patient care to other providers, patients move from one service to another (intra-facility) or from one context to another (e.g., ICU to ward).
- We will highlight gaps to avoid and tricks for efficient transitions.
